Dr. Michalak’s office is located in the N.E. Gilman Professional Center in Issaquah, conveniently located just blocks from I-90 with plenty of free parking.

Our office and surgical suites are accredited and certified, providing patients the highest quality of medical and surgical care as measured by nationally recognized accreditation agencies.

Appointment Scheduling: Patients will be called one day prior to their scheduled appointment time. We ask that patients notify the office 24 hours before their scheduled appointment if they need to cancel or reschedule their appointment. Patients who do not show for their appointment (No Show) may not be able to be rescheduled and will be handled on a case by case basis.

Our office is located in the NE Gilman Professional Center, next door to Boehm's Chocolate Factory. From I-90 take the Front Street Exit and turn left (east) onto NE Gilman Blvd. Our office is approximately 1/2 mile, off to the right.
